Dynamic Question Set: The WellSuite IV HRA uses branching logic to guide seniors through an intuitive and interactive question set. They see only questions that are relevant to them. The HRA takes 20 minutes or less to complete.
Content-Rich Personal Report: After completing the assessment, the participant receives a Personal Report. This gives an overview of health with easy-to-understand language and an engaging format – with color coding, graphs, icons, and a number-based scoring system. A series of configurable “Next Steps” or “Recommendations” buttons throughout the report helps direct participants to your specific programs and resources. Trending data shows the participant how they are improving from one HRA to the next.
Health Literacy: Our Health Risk Assessment question set and Personal Report avoid medical jargon and are written in plain language. All content is written at a respectful fourth-grade reading level and tested for usability.

The WellSuite IV HRA for Medicare satisfies the requirements for NCQA WHP 5 certification, and enables organizations to pass the requirements for PHM 4, Elements A through G (formerly MEM 1).
