WellSuite® IV HRA for Medicaid
Improve the health of your Medicaid member population
Our NCQA-certified WellSuite® IV Health Risk Assessment (HRA) for Medicaid helps you predict and prevent future healthcare costs for your Medicaid members. It’s a powerful tool for understanding their health status, risks for chronic disease, socioeconomic influencers affecting their well-being, and motivation to make changes for better health.
Our HRA can be completed on a desktop, mobile device, or on paper. With the data you can:
- Learn how members’ lifestyles impact their health and well-being
- Educate members with tips and tools to make healthy choices
- Implement the most effective interventions
- Initiate clinician-patient conversations to address health risks
- Increase member participation and engagement
- Track a member’s progress as they move in and out of your health plan
Wellsource understands that the Medicaid population faces a complex set of circumstances and needs. Unlike a one-size-fits-all HRA, the WellSuite IV HRA for Medicaid asks targeted questions and is written with health literacy in mind. All content is graded at a fourth-grade reading level. It is recognized for its scientific validity.

Inform Population Health Management Strategies with Actionable Data
Assess “readiness to change” to target resources efficiently
Our Health Risk Assessment identifies members’ unhealthy behaviors that they are willing to change, allowing you to focus intervention efforts on areas where you are most likely to have success. Providers, care coordinators, and health plans are able to pinpoint and prioritize the best interventions for each person.
This valuable data can help staff immediately connect the member to resources to address health risks. You can target high-risk members for a care management intervention.
Use the HRA data to measure the effectiveness of your interventions. The first use of an HRA provides baseline data on the health of an individual or group. Administering the HRA over the course of the year gives you the opportunity to track progress over time.
Increase Medicaid Member Engagement
Encourage members to take an active role in their health
It’s not just the data collection that makes the WellSuite IV Health Risk Assessment valuable. Use it to drive member engagement and improve patient experience. Our HRA engages members in their care and helps providers and members work in close cooperation.
- The assessment process encourages members to think about their health and ways to improve.
- A visually engaging Personal Report explains risk factors and simple ways that healthy habits can be incorporated into their daily routine.
- Optional library of how-to articles, quizzes, and mini-assessments offers additional resources for positive behavior change.
- A Physician Summary report helps clinicians develop a care plan with the member.
- A dialogue based on the results of the HRA raises awareness of health risks and creates teachable moments.
- The provider can help with setting goals and guide the member with action-oriented advice, referrals to programs and services, and educational materials.
- Results of subsequent HRAs allow members to see how their health has improved over time.
Intuitive and Easy to Understand
Adaptive design for a personalized experience
Dynamic Question Set: The WellSuite IV HRA uses branching logic to guide seniors through an intuitive and interactive question set. They see only questions that are relevant to them. The HRA takes 20 minutes or less to complete.
Content-Rich Personal Report: After completing the assessment, the participant receives a Personal Report. This gives an overview of health with easy-to-understand language and an engaging format – with color coding, graphs, icons, and a number-based scoring system. A series of configurable “Next Steps” or “Recommendations” buttons throughout the report helps direct participants to your specific programs and resources. Trending data shows the participant how they are improving from one HRA to the next.
Health Literacy: Our Health Risk Assessment question set and Personal Report avoid medical jargon and are written in plain language. All content is written at a respectful fourth-grade reading level and tested for usability.

Robust Reporting Capabilities
Get quality data that’s based on solid scientific research
We understand how important actionable data is to population health management. You can access raw data on all participants and groups in your program 24/7, allowing you to perform deep analysis on your populations and customize your own reports. You can also create aggregate reports for an at-a-glance look at the health of your Medicaid groups.
With push reporting, you will automatically and securely receive a copy of each member’s Personal Report and a Physician Summary PDF file upon completion of their HRA. The Physician Summary displays a clinical view of a member’s answers to the HRA, and includes their stage of readiness to change. This allows you to home in on key risk factors that the member is ready to address.
