 The Wellsource® Personal Wellness Profile™ (PWP) is the foundation for an optimal health system. First, PWP assesses health risk, then motivates individuals to make improvements in their health and lifestyle while providing the wellness administrator with essential information for directing follow-up and evaluating outcomes.
The online version of PWP gives your employees the opportunity to take the health risk assessments (HRA) at their convenience throughout the year. In addition, the administrator has 24/7 access to the aggregate group reports simply by logging into our secure severs through any web browser.

PWP Editions
- Comprehensive Plus*
PWP - Comprehensive Plus Edition provides a thorough wellness-based health assessment addressing all major lifestyle factors, personal and family history, symptoms, functional health status and quality of life, health interests, readiness to change, and self-efficacy. Participants commonly take 25-30 minutes to answer 75 question. Extra time is needed for completing optional clinical and fitness data.
- Concise Plus*
PWP - Concise Plus Edition is a condensed version of PWP Comprehensive Plus Edition, and is used in settings where a shorter, simpler questionnaire is needed. The data produced by the PWP Concise Plus Edition also assists in the development of lifestyle improvement programs, health enhancement, and prevention services to address NCQA quality criteria.
Individual Reports
For the online version of PWP, the questionnaire is simple and straightforward.
Employee submits the completed Personal Wellness Profile™, a report is calculated within seconds. The report shows risk factors in many categories such as family health history, lifestyle, and lab results.
Links on the report provide the employee information that could help them lower their risk in problem areas. Also, the employee could take this report to their physician for follow-up.

The Summary Group Report provides a brief statistical look at general health, personal health problems, and healthy lifestyle indicators for the total population assessed.

The Detailed Group Report includes all the information in the Summary Group report, plus more detailed aggregate data for populations. Includes all risk areas assessed by the PWP.
