The Wellsource® Personal Wellness Profile™ (PWP) provides the foundation for an optimal health system by first, assessing health risk, then motivating individuals to make improvements in their health and lifestyle while providing the wellness administrator with essential information for directing follow-up and evaluating outcomes. Wellsource has six PWP versions available depending upon your program goals.

PWP Editions
- Comprehensive Plus*
PWP - Comprehensive Plus Edition provides a thorough wellness-based health assessment addressing all major lifestyle factors, personal and family history, symptoms, functional health status and quality of life, health interests, readiness to change, and self-efficacy. Participants commonly take 25-30 minutes to answer 75 question. Extra time is needed for completing optional clinical and fitness data. Extensive personal, aggregate, and administrative summary reports can be produced with virtually unlimited selection criteria for targeting subgroups and outcome analysis.
- Concise Plus*
PWP - Concise Plus Edition is a condensed version of PWP Comprehensive Plus Editon, and is used in settings where a shorter, simpler questionnaire is needed. The data produced by the PWP Concise Plus Edition also assists in the development of lifestyle improvement programs, health enhancement, and prevention services to address National Committee for Quality Assurance (NCQA) quality criteria.
- Senior Edition*
PWP - Senior Edition is a comprehensive health assessment designed specifically for seniors (suitalble for age 55 and older) that provides the foundation for a proactive health management program. An assessment designed for the general population does not always address the specific needs of every population. This exclusive senior-based design means that the assessment data and health enhancement recommendations are built from senior-specific research.
- Teen Edition*
Helping teens develop positive lifestyle habits early in life is an essential goal of any long-term prevention program. The Teen version of PWP can help assess education and reinforce positive lifestyles during these important formative years. PWP - Teen Edition can help your organization become actively involved in improving the health of the youth in your community.
- Spanish Edition*
PWP - Spanish Edition is a health assessment and profiling application for managing the health of Spanish speaking populations. The Spanish version of PWP provides you with the same power to assess health risk, target specific groups, track progress, and evaluate cost-benefit as the English PWP - Concise Plus Edition.
- Spiritual Edition*
PWP - Spiritual Edition is the Comprehensive Plus Version of PWP with an additional spiritual health component. It provides a health assessment that includes spiritual health data along with traditional health information for use in health screening and health promotion programs.
PWP Administrative Reports
Summary Group Report provides a brief statistical look at general health, personal health problems, and healthy lifestyle indicators for the total population assessed.
Detailed Group Report includes all the information in the Summary Group Report, plus more detailed aggregate data for populations. Includes all risk areas assessed by the PWP.
Group Progress Report compares results for the previous and current testing sessions.
Physician Summary in a brief, one-page profile shows existing problems, health history, and clinical values, alerting the individual's physician to chronic disease or current symptoms.
Counselors Summary provides a detailed two-page risk factor profile for the individual's health counselor showing clinical test results and health risks in all areas.
Executive Summary prioritizes risk factors and recommendations for health improvement programs based on the needs of the group.
Economic Impact Report* gives a detailed cost benefit analysis that can justify your wellness program. Average claims cost per employee is determined and compared to realistic goals for your group.
Personal Progress/Trend* shows changes in an individual's test results and wellness ratings for each test date. A bar chart shows previous versus current test results.
